I acknowledge that play activities at MonkeyDo Adventurous Play — including climbing, sliding, the ball pit, and sensory equipment — carry inherent risks. I voluntarily assume all such risks for myself and the children in my care.

I agree that children must be supervised by a participating caregiver at all times. Socks are required in all play areas. I will follow all posted rules and staff instructions.

I release MonkeyDo, its owners, staff, and affiliates from liability for any injury, loss, or damage arising from use of the facility, except in cases of gross negligence.

I confirm I am the parent or legal guardian of the children listed in this reservation, or am authorized to sign on their behalf.
